Welcome to Community Care Behavioral Health Organization (Community Care)! We are pleased to have you as a member of HealthChoices. Community Care is part of your Medicaid insurance. If you live in one of the counties we serve, your behavioral health benefits are managed by our insurance plan.

Community Care has been supporting individuals throughout Pennsylvania in their recovery from mental health conditions and substance use disorders for more than 25 years. We partner with Counties and Primary Contractors across Pennsylvania to provide behavioral health managed care in the following areas:
