Getting Timely Treatment After Being in the Hospital

We keep track of how many of our members have an appointment within both 7 days and 30 days after getting out of the hospital. We track these things because we believe that members who have an appointment soon after getting out of the hospital have a better chance of staying out, and a better chance of recovery.

It is important that you have an appointment soon after getting out of the hospital. If you need help getting an appointment, please call us.

Community Care is a proud part of the UPMC Insurance Services Division, which also includes UPMC Health Plan, Workpartners, and UPMC for You (Medical Assistance).
