Annual Member and Family Satisfaction Surveys

What: A mail survey for members and families to rate Community Care and its services for mental health and substance use. The surveys are mailed by an outside company. They make sure that your answers are private.

When, Where: The survey is mailed once a year in February or March to some people who used mental health or substance use services in the last year. You can fill out the paper survey and mail it back, do it online, or do it over the phone.

What You Can Do: To get a survey, call Member Services at 1-800-553-7499 before January 15^th each year. Ask to be added to the Annual Satisfaction Survey Request list. You will be sent a survey when they get mailed early each year in February or March.
